The most common SEO mistakes are keyword shuffling, broken links, copied content, duplicate content, duplicate content in more than one location, links from non credible sources, meta keyword tags, avoiding analytics, not optimized with the right keywords, not allowing your site to be crawled, irrelevant anchor text links, etc.

the most common seo mistakes that I have seen is focusing on head terms in the posts. e.g. SEO keywords, seo tools. such kind of terms. If you can't find the long tail keywords, use my latest post on long tail alternative it will help you finding the easy keywords.
