Make the list of all your back links you want to remove, save it in notepad and upload that file in disavow tool. but don't forget to read the webmaster guide lines before submitting.

login to your google account then go to the Disavow tool then select your site, select disavow links, then you will get the option to upload a file, So you can upload your file then click on submit.

Follow steps and Disavow accurately:
Log in to your Google account. Go to the Disavow Tool. Select your site. Click “Disavow Links” Choose the file you created Submit
