Keyword density calculation
 

When I calculate my keyword density should I be considering my Title tag, Meta tags, and page content?

Or just consider the density of the Content...i.e. I want to hit a 2.5% density but considering all those items above I'm sitting @ 3.79%

i'm guessing it a calculates the occurrence of one or the other keyword rather than both as a phrase, since 3.27 is approximately half of 6.55.
