You should take help from Google keyword tool to check the most effective keywords for your site. Use those with high global searches.

Hmm set your keywords according to your product and after analysis use Google Keywords Tool. It will help you to choosing best keywords for your product

Hmmm...there is no particular format but idea is to use them organically (in title/ description/ content etc...). Once you have embedded your keywords, best would be to read it aloud to yourself to see if it does not hinder readability.
Ideally, its one primary keyword and few secondary keywords per page.
