Blog and Forum commenting are effective link building techniques as always but we should have to make post on relevant forums and blogs. Also we should have to vary keywords while we are making anchor text.

It depends on those who comments on a blog. If someone give non relevant comment on a blog post, you can consider it as spam. In this sense yes comment spam is there. If you are a genuine reader, comments have to be relevant to the blog post.

I think there are other mistake in your work, blog commenting may be nofollow but i think that may be not showing spamming.

Yes.when you post i relevant post or start relevant topic against the post then Google consider your link as spam.Be relevant to the post and give proper ans to the people than Google will give importance to your site.
